Growing With the Needs of Our Community
The Woodlands story has grown through a continued commitment to meeting the changing health care needs of Mahoning Valley residents.
Over the years, Woodlands has expanded from skilled nursing services into a broader continuum of care that includes rehabilitation, assisted living, outpatient therapy, home health care, specialized services, and long-term care.
Woodlands at AustinWoods
AustinWoods is a 99-private-suite facility, which includes 24 suite rooms on our Skilled Rehabilitation Unit.
AustinWoods has a Specialized Ventilator Unit with 24-hour-a-day Registered Respiratory Therapists available who work in conjunction with our Speech Therapist. A local Pulmonologist serves as our Medical Director and oversees our Ventilator program and is readily available to meet the needs of our residents.
AustinWoods has opened an In-Patient Dialysis Den in partnership with Fresenius Kidney Care, as well as a local Nephrologist who serves as our Medical Director and oversees the needs of our dialysis residents.
The den serves current residents and gives them the ability to receive dialysis in-house, thereby reducing travel to and from dialysis centers. The benefits of receiving dialysis in-house include providing the resident with more frequent and shorter sessions leading to a faster recovery period. It reduces emergency and hospitalization visits and increases residents’ energy levels.
Woodlands at HamptonWoods
In 2006, AustinWoods was joined by a sister facility, HamptonWoods. During this period of expansion, the Board of Directors created the name “Woodlands” to easily define all services offered throughout the organization.
Woodlands at HamptonWoods was designed using the “Clustered Cottage Concept.” This concept allows residents with similar needs to live in the same “neighborhood.”
HamptonWoods consists of four individual neighborhoods, each having a separate living space and dining area central to the resident rooms. Outside the neighborhoods, residents can enjoy a beautiful living room with fireplace, family kitchen, and porch.
The porch provides accessibility to a beautifully landscaped, secure courtyard, allowing residents safe access to the outdoors. All resident rooms offer private bathroom facilities, most with showers.
Many private rooms are provided at HamptonWoods. Some private rooms offer separate sitting areas, while others offer a shared bathroom.
Woodlands Home Care
Woodlands Home Care provides skilled services in the Trumbull, Mahoning, and Columbiana counties area to clients in their own homes. Care is provided on an intermittent basis, which allows an individual to recuperate from an illness or surgery in the comfort of their own home.
Woodlands Home Care does not provide services for patients under 18 years of age, psychiatric/behavioral health services, ventilator-dependent patients, or patients requiring IV therapy, tracheotomy care, or PleurX catheter drain needs.
Woodlands Home Care does not provide services for patients requiring daily visits when no caregiver is available to the patient.
Our staff of registered nurses, physical, occupational, and speech therapists work closely with your doctor to develop an individual plan of care to fit your needs. The goal is to teach you how to care for yourself, how to manage your illness, while encouraging you to maintain your independence.
Woodlands Assisted Living
The vision of Woodlands did not end at HamptonWoods. In 2010, Woodlands LLC opened Woodlands Assisted Living at HamptonWoods.
Woodlands Assisted Living offers every amenity to make your lifestyle as independent as possible, yet provides the knowledge that further support is always here for you, if and when you need it.
Woodlands Assisted Living is a uniquely designed facility that promotes independence, dignity, privacy, choice, and safety while providing 24-hour care and assistance by a compassionate resident care staff and nursing team.
Center for Rehabilitation
With a strong positive presence in the community, Woodlands Owner, Kathy Prasad, decided to expand Woodlands services yet again in response to the growing health care needs of the community.
Construction began in the fall of 2015 for Woodlands Center for Rehabilitation, a state-of-the-art 26-bed medical rehabilitation facility.
The new building specializes in short-stay rehabilitation and serves patients recovering from surgery, medical procedures, illness, or injury.
Woodlands Center for Rehabilitation features 26 beautifully designed private rooms with hotel-like amenities and focuses on aggressive treatment plans to help patients regain functional mobility and independence.
With the addition of this facility, the Woodlands at HamptonWoods campus offers a full continuum of care to the Valley’s residents: short-stay rehabilitation, respite care, home health care, outpatient therapy, assisted living, and long-term care.
The facility is located just minutes from Mercy Health Hospital in Boardman and is easily accessed off Route 680 on Western Reserve Road.
Moving Forward
With much pride in the past years of accomplishments, Woodlands moves forward, expanding services to Mahoning Valley residents. Woodlands will continue to evolve the company and its services to meet the ever-growing needs and expectations of the community.
